1640 Stridor in An Infant with Double Aortic Arch

Abstract An eleven-month-old infant presented to our casualty with stridor that had been noticed by the parents a few weeks earlier and became increasingly evident lately. They did not recall specific event when “chocked”, but they denied any recent history of respiratory tract infection. The no relevant medical or surgical history, he was born full term normal vaginal delivery. Examination showed moderate biphasic both suprasternal intercostal recessions. Oxygen saturation on room air 94% temperature 37.1°. Chest auscultation revealed bilateral equal entry, though decreased, transmitted stridor. No lung crepitations were appreciated. Suspicion unwitnessed foreign body aspiration arose, decision diagnostic rigid bronchoscopy taken. Rigid along tracheobronchial tree, circumferential constriction lower trachea noted tendency tracheal wall collapse upon withdrawal bronchoscope. Following recovery, computed tomography contrast neck chest done vascular ring, consisting double aortic arch, found compressing narrowing trachea. Further follow up over following 6 months marked improvement his distress, active observation continued. Conclusions Although is commonest cause in infants young children, external compression trachea, particularly rings, should be considered children otherwise unexplained
